PATIENT RESPONSIBILITY AT PT PLUS
If your insurance coverage includes co-payments, co-insurance or deductibles, it will be your responsibility to pay them at the time of service. Our billing staff will be happy to help you understand your benefits and financial responsibilities and help you in any way that they can. If the information you give us is incorrect or inaccurate, you may be held responsible for payment of services rendered. Therefore, it is very important to let us know immediately if your insurance changes so as to better assist you.
PRIVATE & NO FAULT INSURANCE
Please make sure you give us appropriate carrier information so we can process your claim quickly and accurately. We work with a wide variety of Private and No Fault carriers to make things as convenient as possible for our patients.
Medicare patients, by law, are required to see their Primary Care Physician or Referring Provider on a monthly basis. (This is a Federal regulation.) To continue physical therapy at PT Plus a plan of care will be submitted to your physician for his approval.
Please check with your employer’s Workers’ Compensation Insurance carrier to make sure that you have completed the accident report and have filed all the required paperwork in order to collect Worker’s Compensation benefits. It is also important that you let your PT Plus therapist know when you have a follow-up appointment with your referring physician. Our PT Plus therapist will send your physician a progress note regarding your care at PT Plus.
